Questionnaires on Embodia - Creating questionnaires from scratch

Questionnaires allow you to collect standardized information from your patients. You can build them from scratch, edit, and reuse them — including duplicating questions to speed up creation. This guide will show you how to create a questionnaire from scratch. To learn how to create a questionnaire from an existing template in Embodia, please visit this guide.


Create a Questionnaire from Scratch

  1. Go to HEP > Questionnaires, and then click Add questionnaire.

  2. Give your questionnaire a title, choose the questionnaire Type (Outcome measure, Intake form, or Consent form), select a Scoring strategy, and make your questionnaire Active by checking the box next to Active.

  3. Click Submit to create your questionnaire. You will now see an empty questionnaire where you can begin adding items.

  4. Click New item to add items to your questionnaire:

    You can choose from the following types:

      • Instruction: This item type is not fillable by the patient. Use it to provide context or instructions that help patients understand how to complete the questionnaire.

      • Question: This item type is fillable by the patient. Patients will provide their answers directly. Learn more about the available question types in our help article, Question types on Embodia.

      • Connect a patient property: Similar to questions, this option links a patient property to your questionnaire. When a patient answers this question, their response automatically updates the associated patient property on their profile. (Available for Tier 3 memberships only.)   5. Questionnaires automatically save as you build them. When you’re finished building your questionnaire, click Back or navigate to a different page. 


Additional Tips and Features


Should I Connect a Patient Property or Add a Question?

Use Connect a patient property if the information changes infrequently and should be stored as part of the patient’s profile. For example, date of birth, address, or phone number are best added this way — these responses will automatically update the patient’s profile.

Use New Question for all other information that’s specific to a consultation, visit, or a clinical case. For instance, if you want to ask, “What is the reason for booking this consult?”, this should be added as a New question rather than a patient property.

 

Question Types

There are various Question types available including: Free text, Single answer, Multiple answers, Range, Drawing, and File upload. You can learn more about each of these Question types in this guide.

Duplicate a Question

If you want to reuse a question you already added, click Duplicate question:

A popup will appear and you can edit the question as needed. Please note: The Question type cannot be edited.

Reorder Questions

Drag and drop questions to adjust the order of your questionnaire by clicking Change items order:

Preview the Questionnaire

You can also choose Preview to review how it will look to patients:

Manage Your Questionnaires

Once you’ve created questionnaires, you can

  • Edit them at any time

  • Unpublish outdated versions by deselecting the 'Active' checkbox in the edit window

  • Assign questionnaires to consults or patient profiles. 


Send to Patients

You can send questionnaires by attaching them to a consult:

  • Before appointments

  • After appointments

Patients will receive a link and can complete the questionnaire on any device by signing into their secure Embodia portal.

You can also manually send a questionnaire to a patient and complete a questionnaire with a patient.
